Fees And Insurance
Payment for dental treatment is expected at each visit. We accept payment by cash, check, and credit card. Please note that we do not have a sliding fee scale or offer free care.
Your estimated fees will be discussed with you before you begin treatment. RSDM accepts Medicaid plans, which may require a co-pay for each visit and pre-authorization of dental benefits. We do not accept private insurance. After making payment in full, patients with private insurance may apply to their insurance company for reimbursement. Please contact your plan for information about coverage. Medical insurance plans do not usually pay for dental services.
Please bring to your first appointment Medicaid insurance information and five points of ID. A Medicaid card alone is not sufficient identification.
- Photo ID
- Medicaid Insurance Card (if you have insurance)
- List of prescription medications
- Health History Information or Form
- Registration Form
If your insurance company requires prior authorization, please give the form to your student doctor on the day of your treatment plan discussion. You may discuss payment with the Financial Counselors or call (973) 972-5303.
