Missed Appointment & Cancellation Policy
To provide you with high-quality dental care, please arrive on time for your appointment. If you are over 15 minutes late, your appointment may be canceled and need to be rescheduled.
If you need to cancel your appointment, please let us know 24 hours before your scheduled appointment time.
A “missed appointment” occurs when you do not show up for an appointment and do not cancel the appointment 24 hours before the scheduled date/time. We will record this in your medical record as a "missed appointment occurrence.” Patients who have multiple missed appointment occurrences in a 12-month period may be subject to a missed appointment charge.
- 1st occurrence: Our office staff will call you within 24 hours and notify you of your missed appointment.
- 2nd occurrence: Our office staff will call you within 24 hours of your missed appointment, and we may send you a letter reminding you of our policy.
- 3rd occurrence: You will be subject to a $50 missed appointment charge.
We understand that certain circumstances may cause you to cancel your appointments. Our practice firmly believes that a good physician-patient relationship is based on understanding and good communication. We may, at our discretion, terminate our relationship with you if missed appointments become frequent.
This policy is subject to the requirements of applicable dental or health insurance policies and programs.
